This is such an easy project. This is the 3D project I had for my class. They are made with the Two Tags Bigz Die with the Big Shot from Sizzix. What I like about this tag is it has a slit for closing and a score line already made for you when you run the die through the Big Shot. They can be as elaborate or simple as you like. I just a Texturz plate to add the large dots. I put an extra score on mine so it will be a little wider to put a mini candy bar in. See photos below for detail. Just glue dot it to the tags. I am trying to lose weight and can't have milk anyway, so I didn't want to buy any for props. You will just have to imagine them being in there. LOL :-)

Very cute idea, cute tags.
I love this. I was looking at it closely trying to figure it out and when I scrolled to leave a message I see your tutorial. TFS. By the way, with the white stripe your cards look like hockey jerseys.
